Heaven and Hell

While walking down the street one day a Corrupt Senator was tragically hit by a car and died.

His soul arrives in heaven and is met by St. Peter at the entrance.

"Welcome to heaven," says St. Peter. "Before you settle in, it seems there is a problem. We seldom see a high official around these parts, you see, so we're not sure what to do with you."

"No problem, just let me in," says the Senator.

"Well, I'd like to, but I have orders from the higher ups. What we'll do is have you spend one day in hell and one in heaven. Then you can choose where to spend eternity."

"Really?, I've made up my mind. I want to be in heaven," says the Senator.

"I'm sorry, but we have our rules."

And with that, St. Peter escorts him to the elevator and he goes down, down, down to hell.

The doors open and he finds himself in the middle of a green golf course. In the distance is a clubhouse and standing in front of it are all his friends and other politicians who had worked with him.

Everyone is very happy and in evening dress. They run to greet him, shake his hand, and reminisce about the good times they had while getting rich at the expense of the people.
They played a friendly game of golf and then dine on lobster, caviar and the finest champagne..

Also present is the devil, who really is a very friendly guy who is having a good time dancing and telling jokes.

They are all having such a good time that before the Senator realizes it, it is time to go.

Everyone gives him a hearty farewell and waves while the elevator rises..

The elevator goes up, up, up and the door reopens in heaven where St. Peter is waiting for him, "Now it's time to visit heaven...”

So, 24 hours passed with the Senator joining a group of contented souls moving from cloud to cloud, playing the harp and singing.. They have a good time and, before he realizes it, the 24 hours have gone by and St. Peter returns.

"Well, then, you've spent a day in hell and another in heaven. Now choose your eternity."

The Senator reflects for a minute, then he answers: "Well, I would never have said it before, I mean heaven has been delightful, but I think I would be better off in hell."

So St. Peter escorts him to the elevator and he goes down, down, down to hell...

Now the doors of the elevator open and he's in the middle of a barren land covered with waste and garbage. He sees all his friends, dressed in rags, picking up the trash and putting it in black bags as more trash falls from above

The devil comes over to him and puts his arm around his shoulders.

"I don't understand," stammers the Senator. "Yesterday I was here and there was a golf course and clubhouse, and we ate lobster and caviar, drank champagne, and danced and had a great time. Now there's just a wasteland full of garbage and my friends look miserable. What happened?"

The devil smiles at him and says,

"Yesterday we were campaigning, Today, you voted.."

I don't know to whom to contribute this but it seems to wrap it up well...

Let me get this straight......we're trying to pass a health care plan written by a committee whose chairman says he doesn't understand it, passed by a Congress that hasn't read it but exempts themselves from it, to be signed by a president that also is exempt from it and hasn't read it and who smokes, with funding administered by a treasury chief who didn't pay his taxes, all to be overseen by a surgeon general who is obese, and financed by a country that's broke.

What could possibly go wrong?

I have returned from my favorite eye doc meeting in Atlanta, SECO International. I sat through a record for me 16 hours of continuing education. Actually learned a few things!!

Aside from excellent education courses, the meeting has an enormous exhibit hall for us to research and buy new things for the office. Anyone that's been a patient or visited my office knows I have plenty of high tech equipment, so my goal this year was to improve our selection of eye wear. I bought a new line that will be coming in April that I'm keeping under my hat for now but it's going to be HUGE. My guess is I'll have trouble keeping them in stock.

I am considering but struggling with the idea of bringing in Oakley's. They have both sun and regular glasses (ophthalmics) that are super cool. I know they are wildly popular but there's a couple of reasons I've resisted buying them. One, they are owned by Luxottica which also owns Lenscrafters. Is that a good business decision? Second, they would compete with my sunglass line that I feel is optically superior, Maui Jim. Then again, these two lines may be for two distinctly different buyers. Decisions, decisions.

So what do you guys think? Yes or No to Oakley's?

Bad Week

Sorry if anyone has called the office this week for just about anything. Our office software has been completely useless since the snow arrived. We have no way of accessing any of your records, the schedule, billing, etc. Nuttin'!

The software people tell me they hope to have it back up tomorrow. They didn't sound very enthusiastic though. I might not have any employees if they don't fix it soon.

The New York Times (8/5, D3, Markoff) reports that applied physicist Stephen Kurtin, PhD, has invented eyeglasses that "he believes can free nearly two billion people around the world from bifocals, trifocals, and progressive lenses." The eyeglasses "have a tiny adjustable slider on the bridge of the frame that makes it possible to focus alternately on the page of a book, a computer screen, or a mountain range in the distance." Kurtin "has succeeded in creating glasses with a mechanically adjustable focus," which "he says...are better than other glasses and some forms of LASIK surgery." Just "this month, TruFocals, the company he founded three years ago, has begun selling its first adjustable focusing eyeglasses through a small group of optometrists and will soon sell directly online." The "first to become commercially available" in the US, the TruFocal eyeglasses will "sell for $895." They "consist of a lens that is comparable in thickness to that of commercial eyeglasses."

keratoconus news

There is a promising new procedure for those suffering from keratoconus. Corneal collagen cross-linking using riboflavin involves putting vitamin-A drops in the eye and exposing it to ultraviolet light for a period of time. Unfortunately it is not a cure but a means to stabilize the cornea to prevent further damage and possibly eliminate the need for transplant. The procedure improves corneal rigidity by over 300%. While some undergoing the procedure note some improvement of vision, the purpose is to halt the progression of the disease. It is not mainstream in the US but I am following the procedure closely and will bring it to my keratoconus patients when it becomes available.

It is my unfortunate duty to announce that Dr. Kelly is no longer practicing at Battleground Eye Care effective October 31, 2008. I will be assuming care of her patients going forward.

We wish her nothing but happiness and success in the future.

Due to her absence, we will, yet again, be changing office hours. They will be posted here and on our main site, battlegroundeye.com, in the next day or two. I'm still experimenting. It's hard to find the right balance between a happy staff and patient accessibility.

In continuing coverage from previous editions of First Look, the Milwaukee Journal Sentinel (9/10, Uebelherr) reported that the eyeglasses worn by Republican vice presidential candidate Gov. Sarah Palin (R-Alaska) "are making a serious fashion statement." According to Trendhunter.com, "a website that ferrets out the latest crazes across the globe," Gov. Palin's "frames have fueled a 'designer-eyeglasses frenzy.'" And, "Marie Claire magazine declared the specs, by Japanese designer Kazuo Kawasaki, 'a style hit.'" The Kawasaki 704 titanium frames alone are priced at $375, but "by the time the glasses are ordered and finished, the tab can reach $700 to $1,000," opticians say.

We have a couple of exciting new contact lenses in the office. The industry push is to make healthier, more breathable contacts without sacrificing comfort. The new family of materials is called Silicone Hydrogels (SiHy). Some of the better SiHy products are Ciba Focus Night & Day, Acuvue Oasys, and Bausch & Lomb Purevision. These have been around a while now and have been well received.

A new SiHy lens from CooperVision called Biofinity is going over VERY well so far. They have managed to overcome one of the major complaints of SiHy's which is poor comfort for some patients, but still a very breathable lens.

Another new product from Cooper is the Proclear Daily Disposable for the ultimate in health and comfort. These are primarily aimed at dry eye and allergy sufferers. They are a little on the expensive side so we haven't fit many but those that have them love them.

It's that time again

My family and I will be spending next weekend in Atlanta for my annual 'eye tech' hunt. The meeting is called SECO and I feel it's the best meeting in the world for us eye docs. Aside from the usual equipment and gadget research, this year I will be taking Debbie and Lisa along to help find new eyeglass frame lines. We already have an excellent selection with names such as PRADA, Coach, Vera Wang, Kate spade, ProDesign Denmark, Valentino, Silhouette, Nike, Fossil, Guess, Neostyle, kliik (Denmark), Ralph Lauren, Maui Jim sunglasses, Ray Ban, and many others I can't remember off the top of my head.

Australian Teen Loses Sight from Novelty Contact Lens
A 14-year-old girl in Sydney last her eyesight in one eye after wearing a novelty contact lens to a party. She borrowed the lens from a friend who had ordered them from the internet. The teen developed a severe infection and is now awaiting a corneal transplant.

If you have been frustrated trying to contact us since Wednesday afternoon, you aren't alone. Our building took a direct lightning bolt that zapped just about every piece of our electronic equipment. Phones, network, computers - you name it. We were left with only one working phone line. A new phone system was installed Thursday night, but the answering/messaging isn't set up yet. We should be back to normal by Monday afternoon (hopefully!). These were surge-protected devices, by the way. The electric current found its way through the network cables! I had no idea. Moral: Surge protect ALL cables, not just power.

This will come as no surprise to existing patients but to the uninitiated, Dr. Scott is taking the practice to yet another level. For years we have had the ability to capture digital images of the retina (inner lining of the eye) to check for diseases such as glaucoma, macular degeneration, diabetes, hypertension, etc. However, the technology for digital imaging of the outside of the eye has been marginal. That all changes Friday, 7/7/06. I found an excellent camera for external imaging at a conference in Las Vegas a couple of weeks ago. It will be an invaluable tool in monitoring the progression of conditions such as cataracts, conjunctivitis, contact lenses, keratoconus, and dozens of others. Moreover, if you are accompanying a spouse/child/significant other for their examination, you can actually watch, real-time, what I am seeing in my microscope on an LCD monitors. If that doesn't interest you we always have Bugs Bunny and friends ready for viewing on one of the many room monitors. Come and check it all out!!!

ALLERGIES!!!!!

The pollen counts have been at extremely high levels over the past few weeks. Red, itchy, irritated eyes, gritty sensation, contact lens intolerance, etc., etc. The office has been inundated with similar complaints. The newest antihistamine/mast cell stablilizers are more effective at controlling and preventing these symptoms than OTC products. If you use an over the counter drop more often than once or twice a week, you need a prescription drop. Schedule an appointment with Stacey for an allergy evaluation online or at 336-282-2273.

What if I use Renu with MoistureLoc???

Most, if not all, patients we have fit with Bausch & Lomb's Purevision contact lens were advised to use B&L's Renu with MoistureLoc because of comfort issues with other popular solutions. Unfortunately, given the recent association of the fungal infection with Bausch and Lomb solutions we recommend immediate discontinuation of the MoistureLoc variety. What should you use instead? Good question. I personally wear Purevision on occasion and found Aquify to be a comfortable replacement. But the problem there is they had production issues and the solution in hard to find on store shelves right now. Stop by the office and ask a staff member for samples of several brands and decide which will work best for you.

Most importantly, regardless of which contact lenses or solution you use, make sure you are practicing proper hygiene. This includes:

Always wash your hands before using handling your contact lenses, and dry your hands using a lint-free towel.
Carefully and regularly clean contact lenses
RUB the contact lenses with fingers and rinse thoroughly before soaking the lenses overnight in enough multi-purpose solution to completely cover the lens. Ignore the 'no-rub' claims by the solution manufacturers.
Store lenses in the proper case and replace the lens storage case every three months. We have plenty. Come by and ask for one rather than buy one at the pharmacy.
Clean the contact lens case with soap and hot water after each use and keep the case open and dry between cleanings. Clean in the dishwasher periodically for disinfection.
Remember that saline solutions and rewetting drops are not designed to disinfect lenses.
Only fresh solution should be used to clean and disinfect contact lenses. Never re-use old solution. Contact lens solution must be changed everyday, even if the lenses are not used daily.
(I can't believe I even have to type this one)... Never put contact lenses in the mouth or moisten them with saliva, which is full of bacteria and a potential source of infection.
Don't use tap water or homemade saline solutions. Improper use of solutions has been linked to a potentially blinding condition among soft lens wearers.
Never use contacts that have not been prescribed by an eye doctor, such as contact lenses sold illegally over the counter at your local beauty supply shop.

Important Fusarium Keratitis Update:

The Centers for Disease Control and Prevention (CDC) is continuing its investigation on the multi-state outbreak of Fusarium keratitis that may be associated with contact lens use.As of May 9, 2006, the CDC has received reports of 106 confirmed cases, 12 possible cases and 80 cases still under investigation. Sixty-nine reports include insufficient evidence to classify them as cases or carry other non-Fusarium diagnoses. States or territories with at least one confirmed or possible case include: AR, AZ, CA, CT, FL, GA, IA, IL, KS, KY, LA, MA, MD, MI, MO, NC, ND, NJ, NY, OH, OK, PA, PR, TN, TX and VT. States where all cases are under investigation include: IN, MN, MS, NV, OR and RI.
Not all data are available for all confirmed cases.
However, as of May 9, 2006, of the 98 confirmed cases for which CDC has complete data:

5 (5 percent) did not wear contact lenses (no solution used)
93 (95 percent) wore contact lenses
59 (66 percent) reported using any B&L ReNu with MoistureLoc
19 (21 percent) reported using any B&L ReNu MultiPlus
9 (10 percent) reported using any unspecified B&L ReNu
3 (3 percent) reported using any AMO product
4 (4 percent) reported using any Alcon product

*Some cases reported using more than one type of solution; therefore, the solution categories are not mutually exclusive.

Me and Dr. James E. Joy, Associate Professor at my undergraduate alma mater, Marshall University. His excellent vision (pun intended) saw something in me worth pursuing. As a student of his parasitology class, he encouraged me to do independent studies and research in the field. We eventually published a journal article together. His letter of recommendation surely played no small role in my acceptance to optometry school. New Office Manager

We have been so busy lately, Michelle has been overwhelmed trying to be both my office manager and optician. Therefore she has been moved to full-time optician and a new employee has filled the position of office manager admirably. Her name is Kristie. She has lots of practice managing. While I was in optometry school, she was my wife, Justin's mom, and a bread-winning nurse while her absentee husband got educated. She is now the world's lowest paid, salaried employee. But more importantly she is the world's best wife and mommy to Justin and Andrew Scott, little doctors in training.

New Office Toys

I made my annual trip to Atlanta to attend one of the world's largest eye care meetings, SECO International. The main reason for attending this conference is to attend continuing education classes and shop for new office equipment, i.e, "Dr. Scott's toys."

Most of my time this year was spent attending highly specialized updates in diseases of the cornea and retina. Though I am a voracious reader of everything new in the field of eye care, nothing beats attending interactive lectures where information is exchanged and ideas are passed. More entertaining though are the exhibits. Over 50,000 sq.ft. and over 220 companies showing the latest in products, services and equipment. They range from small booths to large hollywoodesque production.

One piece of equipment I am bringing back is a computer generated eye chart. No more projectors, mirrors and blown bulbs. Me being the computer nerd that I am, I plan to take it one step further. The chart essentially a flat panel LCD monitor with randomized letters (no more memorizing T Z V E C L). I will take it one step futher by adding television capabilities so my paitents may watch TV while waiting for me in the room, instead of staring at the wall or reading expired magazines.

My objective is to have your waiting time as minimal as possible, but sometimes delays occur and it is my pleasure to allow you to entertain yourself in the mean time.

Now I just have to find the time to get all this stuff installed...

Scott Family Fun In Atlanta

A fun time was had by all, especially the boys. Thanks to Mr. Home Depot, Bernie Marcus, Atlanta now has the worlds largest Aquarium. There were so many of us optometrists there that they closed it to the public and turned us loose. Did I mention that they WAY underestimated the number of us that would actually attend? Those poor fish will never be the same :)

One of my drug reps from the company Allergan invited me to a very nice Italian restaurant, Veni Vidi Vici, in Midtown Atlanta. Although the server poured me a glass of wine with cork in it, I left with a favorable opinion. The following night we had dinner at a favorite spot, the Hard Rock Cafe.
